Information sent through this website

Appointment requests ask for your name, email address, phone number, and preferred date and time. General inquiries ask for your name, email address, an optional phone number, and your message. These details are used to respond to your request.

Do not submit health-card numbers, dates of birth, home addresses, symptoms, medical histories, or other medical information. This website is not a patient portal. Call the clinic to discuss your care or ask how to provide medical information.

Email delivery and hosting

When you submit a form, the website sends your details to the clinic’s notification mailbox through Resend, an email delivery provider. Appointment requests also generate an email receipt to the address you provide. A receipt confirms the request was received; it does not confirm an appointment.

Vercel hosts this website and processes requests, including technical information such as IP addresses, browser signals, and request logs for delivery and abuse prevention. Form details are not deliberately written to application logs or an application database. Copies of messages remain with the email providers and in the recipient mailboxes.

Hosting and email processing may take place outside Canada. Ordinary email can be forwarded or accessed by people with access to the mailbox. Please use the phone option if you do not want to send your details this way.

Browser storage and external services

To reduce duplicate submissions, this website stores a request identifier, timestamp, and a digest of your submission in your browser’s session storage. It does not store the form’s readable contents there. The identifier is reused for retries for up to 23 hours. Vercel BotID processes browser signals to help detect automated requests.

The contact page includes a Google Maps embed. Loading the map connects your browser to Google. Prescription-refill links open the pharmacy’s separate website, which has its own information practices.

Questions, corrections, and clinic records

This notice describes the website forms. For questions about a submitted message, access or correction requests, retention, or the clinic’s handling of medical records, contact the clinic directly. Do not send medical information in a website inquiry.
